There is no formal restriction on the distance that the ball can be from the body on release. However the other rules previously mentioned will often result in containing the distance. But there is an exception as shown below.

Yes. If you bend over it is very easy for the ball to above the shoulder as shown in the images below. However, the humereus/torso angle cannot be > 90 degrees.
This is allowed as long as the other rules are followed. In the image that follows, note how both feet stay in contact with the rubber.

This is allowed. Very difficult pitch to master. The ball is inside the wrist with a locked elbow on release as shown in the image that follows.

Yes. This is allowed. Again, the ball is not outside the wrist and the elbow is locked as shown in the following images.
This is a common misconception.
The only time the hips and shoulders MUST be square is on the release of the ball.
